อยู่ที่ตำบลบ้านดู่ เดิมชื่อวัดพระธาตุดอยบ้านยาง สร้างขึ้นในสมัยโยนกไชยบุรีศรีเชียงแสนจนถึงสมัยพระเจ้าพังคราช กษัตริย์แห่งราชวงศ์โยนก องค์ที่ 24 ได้มีพระเถระชาวโกศล เมืองสุธรรมาวดี พระนามว่าพระพุทธโฆษาจารย์ได้นำพระบรมสารีริกธาตุจากประเทศลังกามาถวายพระองค์ จำนวน 16 องค์ พระเจ้าพังคราชทรงโปรดให้แบ่งพระธาตุออกเป็น 3 ส่วน ส่วนที่หนึ่งประดิษฐานอยู่ที่พระธาตุจอมทอง ส่วนที่สองอยู่ที่พระธาตุจอมกิตติ และส่วนที่สามนำมาประดิษฐานที่พระธาตุดอยบ้านยาง ต่อมา พ.ศ.2490 พระครูบาคำหล้า สังวโร ได้บูรณะโดยสร้างเจดีย์ครอบพระธาตุองค์เดิมไว้ ภายหลังเปลี่ยนชื่อเป็นพระธาตุจอมสัก
