เกาะมันกลาง ตั้งอยู่ตำบลกร่ำ เป็นเกาะที่อยู่ห่างจากเกาะมันในออกมา ใช้เวลาเดินทางประมาณ 10 นาที เกาะมันกลางมีหาดทรายขาวสะอาด ทรายมีทั้งเป็นสีส้มค่อนข้างหยาบ และขาวละเอียดแบบเกาะเสม็ด จุดเด่นของเกาะคือมีความเป็นส่วนตัว นักท่องเที่ยวสามารถใกล้ชิดธรรมชาติทำกิจกรรมบนเกาะได้ เช่น เล่นน้ำ ดำน้ำดูปะการังน้ำตื้นบริเวณรอบเกาะ เดินเล่นรอบเกาะ และยังมีจุดที่มีลักษณะเหมือนทะเลแหวกเล็ก ๆ มีแนวสันทรายเชื่อมระหว่างเกาะกับกลุ่มโขดหินในทะเล เวลาน้ำลงจะเห็นสันทรายโผล่มา มีลักษณะเดียวกันกับทะเลแหวก ถือว่าเป็นจุดชมวิวที่สวยงามอีกแห่งหนึ่ง นักท่องเที่ยวสามารถพักบนเกาะได้โดยติดต่อแพ็คเกจทัวร์จากบนฝั่ง
